CCTN Exam Content Outline Explained: 6 Domains, 150 Scored Questions and How Pass/Fail Works

The Certified Clinical Transplant Nurse (CCTN) credential is awarded by the American Board for Transplant Certification (ABTC). If you are planning your preparation, the most useful document is the ABTC Candidate Handbook: it tells you how long the exam is, how many questions count and how much each part of the content outline weighs.
The format in numbers
- 175 multiple-choice questions in 3 hours.
- 150 questions are scored; the other 25 are unscored pretest questions used to evaluate future items. You will not know which ones they are, so every question deserves your full attention.
The six content domains
The handbook publishes both the percentage and the number of scored items for each domain:
- Pre-transplantation Care: 9% (13 items)
- Care Immediately Post-transplantation: 26% (39 items)
- Transplantation Management: 26% (39 items)
- Pharmacological Therapeutics: 17% (26 items)
- Education and Discharge: 15% (23 items)
- Professional Responsibilities: 7% (10 items)
Two domains, immediate post-transplant care and transplantation management, account for more than half of the scored exam. Pharmacology is the third pillar. If your study time is limited, this is where it should go first.
How the pass/fail decision is made
The CCTN is reported as pass or fail. The cut score is set through a criterion-referenced standard-setting study: a panel of experts defines the level of knowledge a competent transplant nurse should have. ABTC does not publish a numeric passing percentage, so be careful with any source that promises one. The practical consequence is simple: aim to be consistently strong across all six domains rather than trying to reach a particular number.
How to prepare with the outline
- Map your experience to the domains. A nurse who works mostly on the floor after surgery may know post-transplant care well and still be weak on pre-transplant evaluation or on education and discharge.

- Give pharmacology its own block. Immunosuppressants, their monitoring and their interactions are a large share of the exam and reward focused review.
